This notice explains how Perry Software LLC, doing business through the OAK EMR product and website, handles information collected through this public website, demo requests, SMS preference pages, and the OAK EMR account portal.
Information we collect
We may collect the information you choose to provide, such as your name, business email, phone number, practice name, provider count, current software, requested workflows, and message. For SMS preferences, we may collect a mobile number, consent status, and messaging records. The account portal processes assigned credentials, session information, account status, subscription details, service usage, invoices, and support requests.
How we use information
- Respond to demo, pricing, implementation, support, and account requests.
- Prepare and deliver a tailored product demonstration or written quote.
- Operate, protect, troubleshoot, and improve the website and account services.
- Maintain records of SMS consent, delivery, help requests, and opt-outs.
- Meet applicable legal, security, accounting, and contractual obligations.
Service providers
We use service providers to host and operate parts of the website and business systems. Public contact and consent forms are processed by Formspree. Website files may be delivered through GitHub Pages and related internet infrastructure. Account and application services may use separate hosting, payment, communications, and operational vendors. These providers receive information only as needed to perform services for Perry Software LLC and are subject to their own terms and privacy practices.
Sharing and sale
We do not sell personal information. We may share information with service providers, professional advisers, or authorities when reasonably necessary to operate the service, protect rights and security, complete a transaction you request, comply with law, or address fraud and misuse. We do not share SMS consent or mobile opt-in information with third parties for their marketing.
Cookies and local storage
The public website does not currently use advertising cookies. The account portal may use browser session storage to maintain an authenticated session. Third-party form, hosting, or embedded services may process technical information such as IP address, browser type, device details, and request logs for security and delivery.
Retention and security
We retain information for as long as reasonably needed for the purpose collected, to provide services, keep business and consent records, resolve disputes, and meet legal or contractual obligations. We use administrative, technical, and organizational measures intended to protect information, but no internet transmission or storage method is guaranteed to be completely secure.
Your choices
You may ask to review, correct, or delete information you submitted through the public website, subject to legal and contractual retention requirements. You may stop operational SMS messages by replying STOP. You may also choose not to submit optional form fields.
Customer and patient data
This public website notice does not replace the agreements that govern customer use of the OAK EMR application. If a medical practice uses OAK EMR to process patient information, the applicable customer agreement, implementation scope, privacy obligations, and any required data-protection terms govern that processing.
