SMS Opt-In Evidence
This public page documents the operational SMS call to action used by OAK EMR, operated by Perry Software LLC, so messaging reviewers can verify the consent flow without logging into the patient portal.
Where SMS consent is captured
1. Public SMS preference form
Patients can submit an optional SMS preference at https://oakemr.com/sms-consent.html. The SMS checkbox is optional, unchecked by default, and not required to submit the form.
2. Patient intake, check-in, and portal registration
During intake, check-in, and portal registration, patients provide or verify their mobile number and see the same SMS disclosure before choosing whether to receive operational text messages.
3. Staff-assisted registration
Staff may record the patient preference only after the patient provides a mobile number and confirms the SMS disclosure. Staff can also record that the patient declined SMS.
